New York notary law only requires no blanks in the Acknowledgement.

Dan - the requirement to have any blank lines w/in the body of a document filled in prior to notarization is state-specific. It is not required in MI (was repealed/removed from the statutes), for example.

remember that there can NEVER be any line left blank on any doc that is notarized. Even if they put a line through it or a question mark, or and N/A or none on the lines. Just dont leave one blank spot. If they refuse, then dont notarize it.

First line = borrowers name(s)
Second line = he/she/they

I have a refi closing in 1 hour and for some reason I'm stumped on what to put in the blanks on the Owner's Affidavit - Please help and yes I'm a newbie.
...(date ect...) before me personally came______________(borrower's name I'm assuming)to me known to be the individual described in and who executed the foregoing instrument, and acknowledgement that ___________________ (?)executed the same.
So what are they looking for on the last line? Thanks!
