Quit Claim deed

Going to add to this: Quit Claim Deed will do it, but some states do provide for other means. I know in CT a Change of Name Certificate could be completed and filed and that would do the trick. Down here in FL we need a QCD to do that.

Because of this lack of warranty, quitclaim deeds are most often used to transfer property between family members, as gifts, placing personal property into a business entity (and vice-versa) or in other special or unique circumstances. Quitclaim deeds are rarely used to transfer property from seller to buyer in a traditional property sale; in most cases, the grantor and grantee have an existing relationship or is the same person.

Quit claim is the simpler of the two and does the job.

No KEN!!! We are not changing from one person to another person. We are changing from one NAME variation of one person to another NAME variation of the SAME PERSON on Title.

Still the quitclaim? or a grant deed?
Signature affidavit is not the solution in this case since it is for changing the name on title.

Quit Claim Deed - transferring ownership from person A to person B.

In this case it's really the same person; just a correction of spelling for the vesting on the Title.

A change of name certificate pursuant to 27 V.S.A. § 350 would do; I don't know what other approaches might work. See http://www.leg.state.vt.us/statutes/fullsection.cfm?Title=27&Chapter=005&Section=00350. But of course it wouldn't be my place to suggest what form to use.

A notary goes to a signing. The signer wants to sign under the name Martha Smith, but the documents say Martha G Smith. Martha refuses to sign. The befuddled notary calls the lender and asks what to do.

The lender asks, what name is on the ID?
Martha Gertrude Smith is the answer

The lender says, we can redraw the documents, but first we need to change the name on Title.

Q. What document is used to change a person's name on Title?
