Journal entries
A journal entry must have:
Date and time of the signing
Document name and document date
Type of notarization i.e. acknolwedgment or jurat
Signer's name and address
Type of identification method used i.e. personal knowledge, ID, or credible witnesses
The ID type, serial #, and expiration date if an ID document is used
Additional notes (optional but encouraged) about anything unusual about the signing. Credible witness info goes in the additional notes.
Signature of the signer
Thumbprint of the signer (sometimes required, but always prudent)
