The signature would have to be within a document, and there must be notarial certificate wording at the bottom of the document, or on an attached form. A signature can be notarized using a Jurat or Acknowledgment. The notary certificate area on notarized documents must have the signature of the notary as well as the impression of the notary's official seal. Please keep in mind that some documents might have more than one notarized signature per signer. Multi-page documents often have a notarized signature in the middle of the document as well as at the end.
