Thanks Linda - They did provide the doc. I amended and provided the APA with the document designation section completed which is permissable in MI. Thank you for your response! Business is picking up and am excited to be FT - so are my kids :) Hope all is well at your end!

Did they send you the whole document or just the acknowledgement? They should have sent the entire document for your correction - if you're allowed to do that there (here in FL we can't make any changes or corrections to our certs after the doc leaves our hands - it's a whole new transaction with revisit to signers required).

If I'm reading your post correctly, and all they sent you was an All Purpose Ack, I wouldn't sign that - I'd require the return of the document it goes with and just fix your oversight. I would not provide an APA...

Hi All-
Company alerted me about missed stamp and sent me the all purpose acknowledgement to correct. Filled it out, identified document, the questions is - do I stamp the copy of the affidavit they sent me as well?? Please advise ASAP if possible need to send back. I have not had a return in three years. Thanks!
