I was developing a form for the notary assignment summary which showed pertinent information such as borrower's name and address, lender, loan number, appointment location, date & time package returned, etc plus a section that automatically calculated travel costs and set fees. However, no sooner had I got a first version of it published and someone said: "there's an app for that" - which is partly true - it is called "TripLog" and I highly recommend it. Uses GPA to track mileage and you can enter expenses. Available for both iPhone and Android. I think you might have to pay a fee for uploading more than 10 pictures of receipts to their Cloud service and perhaps another fee if you want to email yourself quarterly reports and year end reports but it is hopefully nominal.

You may still want my form since it is something that is online, can be printed, or download it to your laptop. I don't charge anything.
