Utah Notary Fees
Acknowledgments - $5 per signature
Certified copies - $5 per page certified
Jurats - $5 per signature
Oaths and Affirmations without signatures - $5 each
A notary may charge a travel fee that does not exceed the state mileage rate if the notary 
agrees prior to the notarization with the customer, or has it written on a sign visable 
to customers.
Immigration forms - $5 per set

General information
Please call 800-620-5802 to begin your Utah notary application process.  They will need your email 
address in order to send you more information about testing.  The governor's office will give you 
a PDF study guide which is what you need to carefully review prior to your online examination. Your 
test results will be mailed ( U.S.P.S mail ) to you the following day.  The exam fee is $30, and the 
exam is 50 minutes long. You will need a $5000 surety bond. 
 
Procedure
Mail the Utah notary application, bond, and test scores, a photocopy of your birth certificate, 
passport, or green card, and a check for $30 payable to the state of Utah.  Note - if you have a 
green card, it must not expire during your four year notary commission term.

The Notary Office will return to you a "Certificate of Authority of Notary Public which you will 
need to get your stamp manufactured.
