Please refer to the forms below as a reference for Acknowledgment and Jurat wording for the state of Arizona.

Please be advised that state notary certificates vary from state to state. 
The Arizona Notary Public acknowledgment certificate is a bit shorter than 
California's, but has all of the critical verbiage.  Acknowledgment 
forms for corporations are not available in all states, but Arizona has one. 
It may not be so easy to verify the official corporate capacity of a 
corporate officer.  If they have or don't have paperwork to prove their 
job title, you can document that in your journal.  Please also note that 
the new Arizona Jurat wording incorporates an affirmation into the Oath 
procedure while these acts were previously separate acts.  Many states are 
now combining Oaths and Affirmations into a single act with the term 
(or affirmed) in parentheses to make it an option. Many individuals don't 
like being coerced into "swearing" under oath since it violates their 
religious beliefs.  Before you become an Arizona notary, you should be 
familiar with these forms, and familiar with filling the forms out.  Make 
sure the signer's signature matches the one on their identification and 
make sure the signer signs your journal.  Don't forget to administer an 
Oath with every Jurat.  The signer should ideally raise their right hand and 
swear under oath that the contents of the document in question are true, 
complete and correct.
